Health Services- Long Term Care

Status: Regular Full Time

Salary Range: $71,698-$89,624 per annum plus comprehensive benefits

Location: This position is a mobile position working on a team that supports 23 Long Term Care homes across the Central West region. This includes Dufferin, Caledon, Brampton, Malton and parts of Etobicoke

Hours of work:35 hours per week 


Who we are: The Long-Term Care Division requires staff to provide person-centered care to our residents and families following the Butterfly Model of Care. This will ensure high levels of emotional wellbeing and a better quality of life for those living in our homes.

 

Staff are to have emotional intelligence ability to deliver person-centered care in an authentic way that is meaningful to the resident and their family.

 

Position Summary:

The Behavioural Supports Ontario (BSO) Mobile Clinical Behavioural Support works within the Behavioural Supports Ontario framework to enhance the quality of life for residents with responsive behaviors and personal expressions in LTCHs across the Central West geographical boundaries, including regional and non-regional homes.

 

The BSO Mobile Clinical Behavioural Support is part of an interdisciplinary mobile outreach team that works alongside LTCH staff to deliver person and family-centred care. Using evidence-informed clinical best practices, which recognize the uniqueness of each person (i.e. personhood), the BSO Mobile Clinical Behavioural Support supports timely interventions and transitional management for residents. They enhance LTCH staff capacity through coaching, mentoring and modeling strategies and interventions to manage and care for residents with responsive behaviours.

 

 

What you will do in this role:

  • Demonstrates commitment to BSO’s mission, values, and provincial framework
  • Collaborates with health care providers and BSO network to reduce, prevent, and manage responsive behaviors
  • Prioritizes referrals, assigns caseloads, and schedules assessments with LTCH and mobile BSO staff
  • Advises and supports referrals to additional health resources for resident care
  • Involves residents and families in care planning, focusing on their preferences and goals
  • Assesses responsive behaviors using a holistic approach, considering individual personhood and environmental factors
  • Identifies and demonstrates non-pharmacological, person-centered strategies and coaches staff
  • Utilizes tools like PIECES, GPA, Cornell, GDS, and others in assessments and collaboration
  • Supports Montessori© or behavior reduction activities to improve quality of life and reduce responsive behaviors
  • Builds staff capacity in assessing, applying, and documenting care strategies
  • Ensures ongoing evaluation and communication of strategies to staff through various channels
  • Supports resident transitions across health care sectors and ensures care strategies are shared
  • Identifies residents at risk for responsive behaviors and suggests appropriate referrals
  • Communicates effectively with leadership, staff, families, and other providers
  • Uses Change Management principles for educational communication and program priorities
  • Participates in ongoing education and training related to responsive behaviors and best practices
  • Attends BSO Network meetings and supports process/policy improvements
  • Follows safety procedures and complies with legislative requirements
  • Responds to emergencies (e.g., bomb threats, fire, missing residents) as needed, at LTC home’s request

 

What the role requires: 

  • College diploma or undergraduate degree in health sciences.
  • Registration with a professional college (e.g. Registered Practical Nurse, Occupational Therapy, Social Work, Registered Nurse)
  • Minimum of 3 years experience in Long Term Care
  • Vulnerable Sector Police Check
  • TB Skin Testing
  • Sound knowledge and application of practices that support individuals with responsive behaviours and personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions
  • Experience in Mental Health or related practices
  • Knowledge of the Fixing Long-Term Care Act, 2021 and other applicable regulations and/or other legislation that is relevant to the scope of practice
  • CPR certification designed for health-care professionals and standard first aid
  • Experience and expertise in training and coaching staff
  • Ability to travel to the various Long Term Care Centres across the Central West geography
  • Valid “G” licence in good standing and access to a vehicle

  • Good problem solving skills
  • Excellent interpersonal skills and conflict resolution skills
  • Must have effective oral and written communication skills in English 
  • Must be able to communicate appropriately with residents, staff, professionals, visitors, and volunteers 
  • Strong organizational and time management skills

 

Nice to have:  

  • Change Management knowledge, skills and training an asset
  • Training in P.I.E.C.E.S© , U-First, Gentle Persuasive Approach (GPA), DementiAbility, and Montessori© based programming are considered assets
