Salary Range: $46.00 to $54.59 per hour as per the collective agreement

The School Health Program requires a Public Health Nurse who can build and maintain strong relationships to deliver population health strategies to improve health outcomes for a group of schools across Peel Region. The individual must work in a self-directed manner within a collaborative team environment to promote and deliver services using a comprehensive school health approach. Please find team specific responsibilities below:

• Establish and maintain relationships with internal and external partners to identify and meet the needs of Peel’s school-aged children
• Take the lead role to work comprehensively with school communities on evidence-based initiatives
• Use theories and principles of health promotion to achieve program outcomes
• Use health promotion strategies to create supportive environments for healthy eating, physical activity, substance use and mental well-being
• Support and provide consultation and guidance to stakeholders
• Experience using Teams, SharePoint, Excel
• Candidate will be required to have access to a car and travel to office and various school sites across Peel Region

What you will do…

• Apply knowledge regarding concepts of population health status, inequities in health, determinants of health, and strategies for health protection and promotion to guide practice
• Utilize research and evidence and apply critical appraisal skills to inform policies, programs and practice
• Provide health information, teaching and counselling to clients
• Use a holistic approach to assess the health status and functional competence of clients with consideration of the impact of the determinants of health
• Identify and analyze how clients' culture, attitudes, beliefs and feelings about health influence interventions
• Establish mutually determined goals with client, including outcome indicators and evaluative measures
• Facilitate client engagement in identifying and defining health issues using community development and community assessment skills
• Participate in the planning, developing, implementation and evaluation of community wide and site-specific public health strategies using Evidence Informed Decision Making (EIDM)
• Advocate for and contribute to the development of healthy public policy
• Collaborate with partners to identify and address public health issues and service gaps in order to achieve improved health outcomes
• Develop, foster and facilitate inter-sectoral partnerships and networks using team building, negotiation, conflict management and group facilitation
• Demonstrate culturally sensitive approaches and strategies with diverse populations
• Communicate effectively with internal and external stakeholders
• Promote awareness of the Region of Peel and Public Health services
• Participate in research, quality assurance initiatives, and mentor-ship opportunities as appropriate
• Adhere to public health and nursing ethical standards and principles
• Fulfill legal and professional obligations and responsibilities in accordance with relevant legislation and departmental policies, procedures and medical directives/acts as an ambassador for the Region of Peel, Public Health Services and Public Health Nursing
• Perform other duties as assigned which support the organizational goals
• Respond to public health emergencies, significant events and incidents as required by the Health Protection and Promotion Act and Emergency Standards

What the role requires:

• A Baccalaureate Degree in Nursing or equivalent recognized by a Canadian University
• Current license to practice as a Registered Nurse by the College of Nurses of Ontario
• Current CPR certification, Level C or HCP, is a requirement for Public Health Nurses in Family Health
• Experience in public health or community health nursing is preferred

• Ability to apply appropriate theory and learning principles to practice
• Knowledge of Peel’s diverse community and associated health impacts
• Demonstrate a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias

Nice to have:
 

• Experience with policy and procedure development and update  
• Experience with CD On-Call and/or urgent response  
• Experience with infection prevention and control
• Experience with iPHIS platform

Work Mode & Job Location: at 7120 Hurontario Street, Mississauga, along with the ability to work offsite. A minimum of two days per week on-site is expected, with additional in-person presence as operational needs evolve. Please note you will be required to travel daily to various community sites, including schools, meetings and resource pick ups in Peel Region (Mississauga, Brampton & Caledon).
